Structured Abstract Introduction: The growing proportion of older adults and ongoing changes in urban structures highlight the necessity of evaluating cities in terms of age-friendly environments and improving the quality of life for this population. The age-friendly city framework developed by the World Health Organization provides a basis for assessing urban settings across physical, social, and health dimensions. This study aimed to evaluate the status of Gonabad based on age-friendly city indicators from the perspectives of older adults and urban managers. Methods: This descriptive–analytical cross-sectional study was conducted in 2021. The study population consisted of older adults living in Gonabad and managers involved in elderly-related affairs. A total of 395 older adults were selected through systematic random sampling, and 55 managers were included through a census approach. Data were collected using a standardized questionnaire based on the eight domains of the WHO age-friendly city framework. The validity of the instrument was confirmed by experts, and its reliability was verified with a Cronbach’s alpha coefficient of 0.76. Data were analyzed using appropriate statistical tests in SPSS. Results: The mean age of the older adults was 66.6 years, and the mean age of the managers was 42.8 years. Findings showed that 83% of older adults and 90.9% of managers evaluated the physical environment of the city as unfavorable. Moreover, 90% of older adults reported the social environment as poor, whereas 80% of managers considered this dimension favorable. In the health dimension, 86% of older adults and 50% of managers expressed negative perceptions. Despite health insurance coverage in more than 95% of older adults, the level of awareness regarding municipal health budgets was low in both groups. Statistically significant differences were observed between the views of older adults and managers across most indicators. Conclusion: The results indicated that Gonabad shows considerable gaps in many age-friendly city indicators compared to desirable standards. Urban planning aligned with WHO guidelines, strengthening physical and social infrastructures, and enhancing managerial awareness of the needs of older adults may contribute to improving conditions and moving the city toward an age-friendly model.
